Welcome to Elevated Psychiatry. These Terms and Conditions (“Terms”) outline the rules and guidelines for accessing and using our website and psychiatric care services. By using our services, you agree to comply with and be bound by these Terms. If you do not agree, please discontinue use of our services.

Medical Services Disclaimer

Elevated Psychiatry provides virtual mental health care through licensed psychiatric providers. Our services are not a substitute for emergency medical treatment. If you are experiencing a medical or mental health emergency, please call 911 immediately or go to the nearest emergency department.

Eligibility

To access our services, you must be at least 18 years old or have parental/legal guardian consent. Our providers may only offer care to individuals physically located in states where they hold active licensure.

Privacy and Confidentiality

Your privacy is our priority. All health-related information is protected under the Health Insurance Portability and Accountability Act (HIPAA). For detailed information on how we collect, store, and use your personal and health information, please review our Privacy Policy.

Quiz Participation and Text Message Consent

By completing any quiz, assessment, or screening tool offered through our website—including but not limited to quizzes related to ADHD testing, functional impairment, and understanding your symptoms—you consent to receive text messages from Elevated Psychiatry. These messages may include follow-up information related to your quiz results, appointment reminders, educational resources, and information about our services.

You may revoke your consent to receive text messages at any time by replying STOP to any message you receive from us or by contacting our office directly using the information provided below. Revoking your consent to receive text messages will not affect your ability to access our other services. Message and data rates may apply depending on your mobile carrier and plan.

Appointments and Cancellations

We ask that you attend all scheduled appointments on time. If you need to cancel or reschedule, please notify us at least 24 hours in advance. Missed appointments or late cancellations may be subject to a fee, as outlined in our appointment policy.

Billing and Payment

Private-Pay and Reimbursement Information

Elevated Psychiatry is a private-pay membership practice. We are out-of-network with all commercial insurance plans, and we do not accept Medicare, Medicaid, or Medi-Cal.

After each visit we provide a superbill that you can submit to your insurer through Reimbursify. Many plans with out-of-network mental health benefits reimburse a portion of the visit fee. Reimbursement is determined solely by your plan and is not guaranteed by Elevated Psychiatry.

Every patient receives a Good Faith Estimate of expected costs before their first appointment, as required by the federal No Surprises Act.

Government Payer Eligibility

Elevated Psychiatry is unable to accept patients who have Medicare, Medicaid, Medi-Cal, or both Medicare and Medicaid. If you are covered by any of these programs, please contact us and we will help you find an appropriate referral.

What Membership Includes

Each plan card and linked plan page lists its included visits, messaging, medication support, assessments, guides, forms, and letters. Secure patient-portal communication and HIPAA-secure texting are included. Routine refill coordination is included when medication is clinically appropriate. Letters and paperwork without added fees are a Concierge benefit.

How It Works

Request an appointment. Use the secure booking link to choose the membership path that fits your current needs.
Confirm eligibility. We confirm your location, payer eligibility, and the appropriate plan before care begins.
Review your costs. You receive secure intake forms and a Good Faith Estimate before your first appointment.
Meet your provider. Your first appointment focuses on history, goals, treatment options, and next steps.

Cancellations and No-Shows

Please provide at least 24 hours’ notice to cancel or reschedule. Missed appointments and late cancellations may be subject to a fee under the appointment policy provided during intake.

Use of the Website

You agree not to engage in unlawful activity or attempt to gain unauthorized access to any portion of our site. We reserve the right to restrict or revoke access for any violations of these Terms.

Telehealth Consent

By using our services, you consent to receive care via telehealth technology. You understand that telehealth may have limitations compared to in-person care, such as possible technical disruptions or privacy considerations.

Limitation of Liability

Elevated Psychiatry is not responsible for any direct, indirect, incidental, or consequential damages related to your use—or inability to use—our services, including issues stemming from telehealth platform interruptions or failures.

Modifications to Terms

We reserve the right to update or modify these Terms at any time. Any changes will take effect immediately upon being posted on this page. Continued use of our services after changes are posted will indicate your acceptance of those changes.

Governing Law

These Terms are governed by the laws of the state where our principal office is located, without regard to its conflict of law provisions.
